Instructions for Getting Signatures on
Midcoast Health Care Reform's Citizens'
Inititiative Petition
The Petitioner (the person signing the petition) must
Be a Maine Resident and Registered Voter.
Sign name as it appears on the voting list.
Sign only once.
Not sign another person's name.
Print name, date of signing, street address (Not P.O Box or RFD numbers) and municipality of residence (unless the municipality is pre-printed by petition circulator).
The registrar will forward the completed document to the Secretary Of State.
The Petition Circulator (the person witnessing petition signatures) must
Be a Maine Resident and Registered Voter.
Complete the circulator's verification/oath at the bottom of the petition in front of a Notary Public.
Make a copy of the notarized petition(s).
Submit the original notarized petition to the city/town registrar for certification.
The registrar will forward the completed document to the Secretary of State.
